We know that providing complete documentation may not be possible, but please provide as much of the following as possible and any other information you feel may be important.

●  DD214 (and/or branch of service, service number, time and place of death and circumstances)

●  Birth certificate

●  High school graduation diploma

●  GED

●  College commissioning forms

●  Transcripts

●  Official documents showing service member’s Home Of  Record

●  Other service documentation from official sources, etc., which may
include award citations and nomination recommendations by unit
members who served with the prospective honoree during
operations.

Please note any misspellings or other errors on the Fallen Hero listings and submit correction information using applicable documents from the above list.

All requests will be reviewed by the Honors Resolution Committee.    All requests will receive notification of receipt and notification of decision.
